Part 3/3 of #46.

I don't know javascript so there is an outrageous amount of duplicate code here.

  • Cloned the submission pages for mapfixes
  • Created /maps/[mapId]/fix to submit mapfixes (/maps/[mapId] page does not exist yet, see #42)
